Printer-Friendly Survey Sections

The following PDF documents correspond to sections of the online survey. These versions of the survey can be printed and used to collect data prior to completing the online survey which should be available on or around October 1st.

You will receive an email with a link and your access information to the online survey when it is ready. There is a slight chance that survey release could be delayed or that minor changes could be made to the wording. If this is the case, we will notify schools, provide a revised PDF and/or adjust the time window for online administration accordingly.

NOTE: All nursing schools need to review and complete information in the Institutional Information and Prelicensure Programs Survey in addition to their program-specific survey(s).
